Description: Mercy Home Health, a member of Trinity Health at Home, is seeking a Full-time Registered Nurse Case Manager for the greater Delaware County area.

Position Overview

Provides primary nursing care to home-based patients as ordered by the physician, in accordance with policies and procedures. Responsible for initial assessments, care planning in collaboration with the Population Health Case Manager, and providing functional support to Home Healthcare Aides and LPNs. Focuses on therapeutic intervention, care management, and achieving high patient satisfaction and outcomes.
