UP Health System HomeCare is seeking a dedicated Physical Therapist to provide in-home care, assessing mobility, endurance, and pain to drive tailored treatment plans. You will deliver hands-on therapy, monitor progress, and coordinate with physicians and nurses to optimize patient outcomes.

The role emphasizes one-on-one patient relationships, flexible scheduling, and meaningful impact on patients’ independence and quality of life in their homes.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsabilités

• Evaluates patient functional status and PT needs; assists in development of therapy plan of care.
• Provides services within the scope of PT practice and plan of care in coordination with health care team.
• Observes and reports patient responses to treatment and changes in condition to supervising nurse/physician.
• Treats patients to relieve pain, develop/restore function, and maintain maximum functioning.
• Instructs patient, family, and care staff about plan of care and progress toward goals.
• Visits patients according to Plan of Treatment; completes progress notes per policy.
• Completes all evaluations and develops PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
