Job summary

Centers Plan for Healthy Living is seeking a Home Care Relations Liaison in New York. The role collaborates with internal departments and external providers to address member provider-change requests and assist with CDPAS documentation and authorizations.

Responsibilities include coordinating with Utilization Management, Case Management, and G&A, communicating with current providers, and documenting actions in CaseTrakker, along with other duties as assigned.

Qualifications

• At least one year of professional experience in related field.
• MLTC knowledge and experience preferred.

Responsibilities

• Serve as the point of contact for Utilization Management, Case Management and G&A to resolve member provider change requests.
• Communicate with current provider regarding stabilizing services.
• Communicate with providers regarding staffing new cases.
• Assist delegating CDPAS documentation to appropriate departments.
• Correspond with external providers regarding CDPAS approvals and authorizations.
• Documentation of all actions in CaseTrakker.
• Other assignments and duties as needed.
