Ability360 in Glendale, AZ is seeking a detail-oriented Home Care Program Supervisor to oversee consumer intakes and assessments, match and monitor Personal Care Assistants, and ensure compliance with AHCCCS and EVV requirements. This is a full-time position at our Glendale office.

Qualifications

• Knowledge of EVV systems and AHCCCS health plans.
• 3+ years’ combined education/experience in case management or home care supervision.
• Bachelor’s Degree preferred for this role.

Responsibilities

• Conduct consumer assessments and supervise on-site visits.
• Assign PCAs based on skill and consumer needs.
• Monitor PCA performance and provide training.
• Maintain accurate documentation and communication with case managers.
• Support conflict resolution between consumers and PCAs.
