Job summary

Enhabit Home Health & Hospice is seeking a Licensed Physical Therapist Assistant to join our in-home care team in Illinois. Under the supervision of a Physical Therapist, you will help patients improve mobility and daily living activities, delivering care in patients' homes with a focus on safety and quality outcomes.

You must hold an active PTA license, have a valid driver’s license, and reliable transportation.

Qualifications

• Must be a currently licensed PTA in the state of employment.
• Must possess a valid state driver’s license.
• Must possess automobile liability insurance.

Responsibilities

• Provide PTA services under the direction of a Physical Therapist to improve mobility and function.
• Assist patients with daily living activities and self-care.
• Support treatment plans to help patients progress and recover in-home.
• Maintain patient safety and professional documentation as required.
