Metro Physical & Aquatic Therapy is seeking a motivated and passionate per diem Physical Therapist to provide in-home care across the Eastern Suffolk County area. If you are a Physical Therapist ready to advance your career, we want to hear from you and welcome new graduates with proper training.

All levels of experience are encouraged to apply; we will provide mentorship for documentation and compliance, with flexible scheduling and the option to see patients seven days a week in your preferred

Qualifications

• Licensed Physical Therapist in New York or eligible.
• Home health experience is preferred but not required; mentoring available.
• Willingness to travel to patients’ homes within designated geographic area.

Responsabilités

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, HHAs, and caregivers on therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate with patients, families, and team members around discharge planning.
• Deliver care in compliance with agency policies and professional standards.

Responsibilities

• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

Additional Information

The anticipated pay range for this position is $70-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
