Job summary

Metro is seeking a motivated per diem Physical Therapist to provide in-home care in Dix Hills and surrounding towns. All levels welcome; new graduates will be trained. You’ll evaluate patients, create treatment plans, and deliver hands-on therapy in patients’ homes with ongoing physician and family collaboration.

Flexible scheduling with 1099 and W-2 options, competitive compensation per visit, and a supportive team environment that emphasizes patient care and professional growth.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - mentor provided for documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

The anticipated pay range for this position is $70-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
