Job summary

SportsMed Physical Therapy is seeking a Home Care Physical Therapist to provide skilled PT services in patients’ homes across New Jersey. You will design individualized treatment plans, promote independence, and collaborate with a supportive team to ensure optimal outcomes.

The role offers a flexible schedule with both full-time salaried and paid-per-visit options, a strong referral network, and opportunities for growth within a rapidly expanding Home Care division.

Qualifications

• Active New Jersey Physical Therapist License is required.
• Medicare documentation and billing experience is preferred.
• Strong communication and time management skills.
• Comfortable working independently.
• Computer literacy.
• Current CPR Certification.
• Professional liability insurance.
• Ability to wear PPE as needed.
• Valid Driver’s License.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Deliver skilled physical therapy services in the home setting.
• Build meaningful patient relationships through one-on-one care.
• Complete documentation accurately and on time.
• Coordinate schedules and communicate with the clinical support team.
• Drive clinical initiatives focused on delivering exceptional patient outcomes.

Compensation

Full-Time Salary: $90,000 – $110,000 Base Salary + Benefits

Paid-Per-Visit Option: $65 – $80 per visit
