Job summary

SportsMed Physical Therapy in New Jersey is seeking Home Care Physical Therapists to help patients regain mobility in their homes while enjoying flexible scheduling and strong support. You will evaluate patients, develop individualized treatment plans, and deliver one-on-one care within a trusted organization.

This full-time role offers a base salary of $90,000–$110,000 plus benefits, with a paid-per-visit option and opportunities to grow within the Home Care division. Active NJ license required.

Qualifications

• Active New Jersey Physical Therapist License.
• Medicare documentation and billing experience preferred.
• Strong communication and time management skills.
• Comfortable working independently.
• Computer literacy and PPE compliance.
• Current CPR Certification.
• Professional liability insurance.
• Valid Driver's License.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Deliver skilled physical therapy services in the home setting.
• Build meaningful patient relationships through one-on-one care.
• Complete documentation accurately and on time.
• Coordinate schedules and communicate with the clinical support team.

Compensation

Full-Time Salary: $90,000 – $110,000 Base Salary + Benefits

Paid-Per-Visit Option: $65 – $80 per visit
