HOME CARE PHYSICAL THERAPIST (PT)
Per Diem, PRN
Halifax/Ahoskie

Job Description

1. Provides patient care services along with direct treatment and follow-up.
2. Evaluates new patients with physician orders and develops appropriate care plans based on diagnoses.
3. Documents patient care data accurately, thoroughly, and promptly, reflecting patient progress or decline.
4. Submits daily/weekly patient visit schedules to the Scheduler.
5. Communicates with intercompany and intracompany contacts as needed.
6. Discusses reimbursement policies and treatment plans with patients, families, and healthcare team members.
7. Coordinates referrals with the Patient Care Coordinator.
8. Provides education to healthcare professionals, patients, and the public; supervises peers and supports personnel and students in training.
9. Conducts research and assists with new employee orientation as needed.

Job Requirements

1. Bachelor’s degree in physical therapy from an accredited program and current state licensure.
2. Preferably at least one year of therapeutic experience outside of home health.
3. Strong knowledge of physical therapy principles, evaluation, and treatment.
4. Minimum of twelve hours of continuing education annually.
5. Willingness to travel and work under assigned conditions.
