Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist to provide in-home care in Dix Hills and surrounding towns. Flexible schedule with 1099 and W-2 options, training provided for new graduates, and opportunities for professional growth in a growing organization.

Responsibilities include evaluating patients, planning and implementing customized home therapy, and documenting visits. Must be licensed in NY or eligible, with ability to travel to patient homes.

Competenze

• Licensed NY Physical Therapist or eligible to obtain licensure.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred but not required; mentorship provided.
• Ability to travel within the geographic territory.

Responsibilities:

• Accept and treat patients within your designated geographic area
• Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards
  • Physical Therapist Program Graduate
  • Licensed Physical Therapist in the state of New York or eligible.
  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
  • Ability to travel to patients' homes within the assigned geographic territory

The anticipated pay range for this position is $70-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
