Job summary

SportsMed Physical Therapy in New Jersey is seeking Home Care Physical Therapists to deliver one-on-one care in patients' homes. You’ll design individualized treatment plans, perform skilled PT services, and help patients regain mobility and independence while managing your own caseload.

You’ll join a growing Home Care division with strong support, steady referrals, and opportunities to advance within an established organization.

Qualifications

• Active New Jersey Physical Therapist license required.
• Medicare documentation and billing experience preferred.
• Ability to work independently and manage caseload across home settings.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Deliver skilled physical therapy services in the home setting.
• Build meaningful patient relationships through one-on-one care.
• Complete documentation accurately and on time.
• Coordinate schedules and communicate with the clinical support team.
• Drive clinical initiatives focused on delivering exceptional patient outcomes.
