Job summary

SportsMed is hiring Home Care Physical Therapists in New Jersey. You will provide one-on-one PT services in patients' homes, building relationships and improving mobility while managing caseloads independently with strong support from an established organization.

The role offers a flexible schedule, full-time salary ($90,000–$110,000) with benefits, and a paid-per-visit option ($65–$80/visit). Growth opportunities exist within a rapidly expanding Home Care division.

Qualifications

• Active New Jersey Physical Therapist License.
• Medicare documentation and billing experience preferred.
• Strong communication and time management skills.
• Comfortable working independently.
• Computer literacy.
• Current CPR Certification.
• Professional liability insurance.
• Ability to wear PPE as needed.
• Valid Driver's License.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Deliver skilled physical therapy services in the home setting.
• Build meaningful patient relationships through one-on-one care.
• Complete documentation accurately and on time.
• Coordinate schedules and communicate with the clinical support team.
• Drive clinical initiatives focused on delivering exceptional patient outcomes.
