Responsibilities include conducting home assessments, developing goals, administering therapies, and educating clients on home programs while maintaining detailed records.

Qualifications

• Degree in Physical Therapy from an accredited program.
• State licensure as a Physical Therapist.
• Strong clinical and interpersonal skills.
• Ability to adapt therapy plans for home care needs.
• Compassionate, patient-centered approach.

Responsibilities

• Conduct comprehensive home care assessments of clients' physical conditions.
• Develop personalized, goal-oriented PT plans based on assessments.
• Administer therapeutic exercises, manual therapy, and other interventions.
• Collaborate with clients, families, and healthcare professionals for coordinated care.
• Educate clients on home exercise techniques and self-care.
• Maintain thorough documentation of assessments, interventions, and progress.
