Compensation

Full‑Time Salary: $90,000 – $110,000 per year + Benefits
Paid‑Per‑Visit Option: $65 – $80 per visit

As a Home Care Physical Therapist, you’ll work directly with patients in their homes and communities to help them regain mobility, improve function, and maintain independence.

Responsibilities include:

• Evaluate patients and develop individualized treatment plans
• Deliver skilled physical therapy services in the home setting
• Build meaningful patient relationships through one‑on‑one care
• Complete documentation accurately and on time
• Coordinate schedules and communicate with the clinical support team
• Drive clinical initiatives focused on delivering exceptional patient outcomes
