Job summary

Community Care Givers in Michigan is seeking a compassionate Physical Therapist to join our growing home health care team. You will assess patients in their homes, develop individualized treatment plans, and help improve mobility and independence while collaborating with nurses and physicians.

This flexible, independent role offers meaningful patient relationships and a supportive team environment, with opportunities to educate families and contribute to better community health outcomes.

Qualifications

• Current Michigan PT license.
• Graduate of an accredited PT program.
• Strong communication, organizational and time-management skills.

Responsibilities

• Provide physical therapy services to patients in their homes.

Responsibilities

Qualifications

• Current Physical Therapist (PT) license in the State of Michigan.
• Graduate of an accredited physical therapy program.

• Ability to work independently and manage patient visits.
• Valid driver’s license and reliable transportation.
• Previous home health experience preferred but not required.
