Job summary

Elite Home Health Care is seeking a Coordinator to ensure smooth coordination of home care services for patients in the New York area. You will manage a caseload, coordinate between clients and caregivers, and ensure timely, quality care while collaborating with the team on staffing needs.

You will update records, participate in on-call duties, and contribute to policy adherence. Strong leadership, organization, and communication are essential for success.

Qualifications

• Healthcare experience preferred in a home care setting.
• Proficient with Microsoft Office Suite and HHA Exchange.
• Demonstrated leadership and problem-solving abilities.
• Excellent organizational skills and attention to detail.
• Strong written and verbal communication skills.
• Ability to multitask in a fast-paced environment.
• Flexible schedule to meet department and patient needs.

Responsibilities

• Manage assigned caseload and coordinate home care services.
• Coordinate placement of services between clients and caregivers using HHA eXchange.
• Maintain caseload management for timely, appropriate care.
• Attend trainings and meetings to stay updated on policies and procedures.
• Assist team lead with coverage and staffing issues.
• Discuss schedules, availability, attendance with department staff.
• Update patient and caregiver records accurately in the system.
• Verify caregivers’ attendance daily and prioritize no-shows or call-outs.
• Handle inquiries from contractors and clients about reports.
• Address caregiver concerns with management in the employee's best interests.
• Assist with on-call duties as scheduled.
