Job summary

Personal Touch Home Care is seeking a compassionate Occupational Therapist to provide rehabilitative services in patients’ homes across Bronx, NY. This 1099 contractor role offers fee-for-service pay per visit with a flexible daytime and after-hours schedule.

You will assess patients, develop treatment plans, and help clients regain independence and daily living skills in a home setting. NY license and professional registration are required, with preference for home health or geriatric

Qualifications

Must have graduated from an accredited OT program.
Valid NY professional license and registration required.
Home health or geriatric care experience preferred.

Responsibilities

Assess referred patients and determine rehabilitation needs.
Develop and implement individualized treatment plans.
Assist patients to improve daily living and work skills at home.
Provide rehabilitative services to patients unable to travel to a clinic.
Promote mobility, function and independence at home.
Document patient condition and progress.
Educate patients and families on therapy methods.
Complete evaluations, weekly progress notes, recertification and discharge documents.
Collaborate with other disciplines to ensure patient-centered care.
Adhere to confidentiality, HIPAA and Residents' rights.
