Job summary

Personal Touch Home Care is expanding our Special Needs Program and seeks compassionate Occupational Therapists for home-based care in New York. This fee-for-service, 1099 contract role requires a valid NY license and offers pay per visit with flexible daytime and after-school/evening cases.

Join a team focused on improving mobility, independence, and quality of life for pediatric, adult, and special needs patients in their homes. Prior home health experience is preferred.

Qualifications

• Graduate from an accredited OT program and hold a valid NY OT license.
• Home health experience and geriatric care preferred.
• Strong communication and organizational skills for patient-centered care.
• Ability to work in patients' homes with flexible scheduling.

Responsibilities

• Assess referred patients requiring rehabilitation services in home settings.
• Develop and implement individualized treatment plans and goals.
• Provide rehabilitative services to patients who cannot travel to clinic or hospital.
• Document patient condition, progress notes, and discharge summaries.
• Educate patients and families on therapy methods and home care routines.
• Collaborate with physicians and care team to optimize outcomes.
• Maintain confidentiality and HIPAA compliance.
