Job summary

Personal Touch Home Care is seeking compassionate Occupational Therapists to cover home-based cases in Queens, NY, including Astoria, Jamaica, and nearby areas. This fee-for-service, 1099 contracted role offers flexible daytime and after-school/evening hours.

You will assess patients, develop and implement treatment plans, educate families, and document progress, collaborating with a multidisciplinary team to improve mobility, function and independence in patients’ homes.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Must hold a valid NY professional license and registration.
• Experience in long-term care and/or geriatric care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to evaluate condition and functional abilities.
• Develop and administer treatment programs with goals per physician direction.
• Set realistic and achievable goals for patients.
• Document and record patient condition.
• Educate patients and families in therapy methods.
• Conduct evaluations, progress notes, recertifications, and discharge documentation.
• Collaborate with all disciplines for patient-centered care.
• Perform duties assigned by the Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.

Clinicians available to cover these areas are especially encouraged to apply! Personal Touch Home Care is expanding our Special Needs Program and seeking compassionate Occupational Therapist who are passionate about delivering high-quality care in the home setting.

This is fee-for-service position, paid per visit and this is a 1099, contracted position. In order to be considered for this position, you MUST have a valid and active license in the state of NY.

Job Overview:

As an Occupational Therapist with Personal Touch Home Care, you will be responsible for assessing patients' needs, developing and implementing treatment plans, and helping clients improve their daily living and work skills. You will also provide rehabilitative services to patients who are unable to travel to a clinic or hospital. This is focused on improving patients' quality of life by promoting mobility, function, and independence within the comfort and convenience of their own homes.

Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services.
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients.
• Document and record the patients’ condition.
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide the best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation.
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Demonstrates good verbal and written communication and organization skills.
• Prefer two (2) years of appropriate experience as an occupational therapist. Home health experience is preferred.
