Job summary

Home Helpers Home Care seeks a Full-time Nurse Supervisor to lead in-home senior care in Raleigh/Cary and the Triangle. You will perform initial in-home assessments, develop care plans, and coordinate with physicians, families, and the care team to ensure high-quality care.

The role requires an RN license in good standing in North Carolina, strong communication skills, and a commitment to safety and excellence in home health.

Qualifications

  • Willingness to adhere to health and safety standards.
  • Strong social, communication and interpersonal skills.
  • Valid Driver's License and Auto Insurance.
  • Strength to perform the job requirements.
  • Diploma from an accredited RN program.
  • Current RN license in good standing in North Carolina.
  • Computer skills including Microsoft Office.
  • Complete drug screen and background check.

Responsibilities

  • Conduct initial in-home clinical assessments for new clients to determine care needs, risks, and appropriate service levels.
  • Perform scheduled and as-needed reassessments to monitor changes in client condition and adjust care plans accordingly.
  • Develop, implement, and update individualized plans of care in collaboration with physicians, family members, and the care team.
  • Identify and upscale changes in client health status, safety concerns, or care plan deviations promptly.
  • Maintain accurate, complete, and timely clinical documentation in client charts in accordance with state regulations and company policy.
  • Ensure all client records meet licensing, accreditation, and payer compliance standards.
  • Conduct periodic chart audits to identify and correct documentation gaps.
  • Stay current on state and federal home care regulations, ensuring agency practices remain audit-ready.
  • Support internal quality assurance and performance improvement initiatives.
  • Serve as a clinical point of contact for clients and families, addressing questions and concerns about care.
  • Communicate care plan updates, health changes, and recommendations clearly to clients, families, and caregivers.
  • Rotate on-call responsibilities.
  • Provide education to clients and families on conditions, care techniques, and safety practices.
  • Foster trust and satisfaction through responsive, compassionate communication.
  • Build and maintain relationships with external healthcare vendors and partners (e.g., physicians, hospitals, discharge planners, physical therapists, hospice, home health agencies).
  • Coordinate care activities with outside providers to ensure continuity and quality of services.
  • Represent the agency professionally in vendor-facing interactions to support the organization's referral development and reputation.
  • Identify opportunities to strengthen partnerships that generate client referrals and community visibility.

Responsibilities:
Clinical Assessments & Reassessments
