Job summary

All Ways Caring HomeCare is seeking a Licensed Practical Nurse to provide direct and indirect care to clients in their home. You will perform nursing duties per physician orders, support health maintenance, and help clients live independently at home.

The role involves documentation, medication administration, and collaboration with the Nurse Supervisor and care team. A state LPN license and CPR certification are required, with experience preferred.

Qualifications

• Current and unrestricted LPN license in the operating state.
• One or more years of nursing experience preferred.
• CPR certification required and maintained.

Responsibilities

• Provide skilled nursing care per physician orders and plan of care.
• Document nursing actions and inform physicians and family accordingly.
• Administer and monitor medications as prescribed by physician.
• Conduct nursing assessment visits as needed.
• Follow policies and procedures per operation and regulatory standards.
• Complete supervisory visits including plan of care review and caregiver competency assessment.
• Coordinate with Director/Nurse Supervisor and Branch Manager to address health needs.
• Participate in staff meetings and in-services as needed.
