The Home Health Services LPN provides skilled nursing care for patients in their homes under the supervision of an RN. They employ the nursing process to deliver care, including specific treatments, health teaching, preventive screenings, referrals, and advocacy. The LPN ensures proper scheduling of visits based on patient needs, complies with all policies, and coordinates services within the agency and community.
