The Home Health Services LPN provides skilled nursing care for patients in their homes under the supervision of an RN. The role involves employing the nursing process to deliver care, including specific treatments, health teaching, preventive screenings, referrals, and advocacy. The LPN ensures adequate service provision and scheduling based on patient needs, adhering to all Home Care policies and procedures.

Work Location: Baltimore County, Baltimore City, Harford County, Howard County, and/or Anne Arundel County

Work Hours: Must be available from 8:30 am to 5:00 pm; patient visit times are flexible.

Who Should Apply?
