Job summary

Addus HomeCare is seeking a Field Supervisor to conduct on-site client evaluations and develop tailored service plans in coordination with authorization. The role includes visiting clients, reporting daily to the scheduler, and supervising attendants to ensure high-quality care, compliance, and timely documentation.

The ideal candidate will have supervisory experience in healthcare settings, familiarity with regulatory requirements, and the ability to manage multiple tasks with professionalism

Qualifications

• Two years of full-time study at an accredited college or university or individuals with a high school diploma or equivalent may substitute one year of full-time employment in a supervisory capacity in a healthcare facility, agency, or community-based agency for each year of college.
• PHC, MC experience preferred.

Responsibilities

• Conduct on-site client evaluations and develop service plans.
• Complete visits as scheduled and report daily to scheduler.
• Recruit, hire, train, and supervise attendants.
• Complete forms and attach to the daily visit report and submit within 48 hours.
• Maintain confidentiality of clients, staff information, and comply with regulations.
