Job summary

Elara Caring is seeking a Certified Occupational Therapist Assistant to join our team delivering high-quality home-based care. You will follow care plans, visit patients, monitor progress, and collaborate with physicians and the care team to ensure effective therapy and safe exercises at home.

Requirements include a completed OT curriculum, current COTA licensure, at least 1 year of clinical experience, and the ability to lift up to 50 pounds.

Qualifications

• Must have completed an accredited certified occupational therapy curriculum.
• Current, unrestricted COTA licensure in state of work.
• At least 1 year of clinical COTA experience.
• Physical ability to lift up to 50 pounds and assist positioning.

Responsibilities

• Provide occupational therapy services by following the care plan developed by the Occupational Therapist.
• Visit patients regularly and monitor their progress during each visit.
• Report changes in a patient's condition to the Occupational Therapist, physician, and care team.
• Follow physician orders and the established plan of care.
• Teach patients and caregivers home therapy exercises and safe techniques.
• Work closely with the care team to provide coordinated, high-quality patient care.
• Complete accurate and timely documentation for each patient visit.
• Help reduce avoidable hospital and emergency room visits through proactive patient care.
• Stay up to date on healthcare regulations and agency policies.
• Work in a physically active role that may include lifting, transferring patients, and occasional irregular or on-call hours.
