Home Care Coordinator & Field Registered Nurse

The Home Care Coordinator/Visit RN is a split role and plays a critical role in developing and managing home care plans for patients referred from hospitals, rehabilitation centers, or skilled nursing facilities. This position operates on a rotating schedule of seven days on and seven days off, totaling 56 hours per pay period.

Key responsibilities include:

Weekend Duties (Saturday-Sunday): Conducting assessments and screenings to determine the appropriateness of home care services, managing referrals, and visiting facilities daily.

Weekday Duties (Monday-Friday): Completing patient visits in the field and ensuring the continuity of care for all assigned clients.

This position requires a dedicated and flexible nurse who can seamlessly transition between coordinating care plans and providing direct patient care in the field.

Performance Responsibilities and Standards Home Care Coordinator:
  1. Promote continuity of care for referred clients.
    1. a. Determine client/family home care needs upon referral for home care.
    2. b. Assist in the development of a home care plan, including the arrangement of needed services, supplies and appliances.
    3. c. Review home care policies and procedures with clients/families, physicians, agency care providers, insurance providers and service and equipment vendors as indicated.
    4. d. Act as a liaison between Nascentia Health and hospitals, rehabs and/or SNF.
  2. Promote quality assurance as it relates to home care planning.
    1. a. Document assessment and home care eligibility per Nascentia Health policies and procedures.
    2. b. Act as a resource and/or consultant to hospital or SNF staff regarding home health services.
    3. c. Participate in quality improvement activities relating to home care planning and coordination.
    4. d. Maintain accurate and up-to-date statistics regarding home care referrals.
    5. e. Participate in team conferences between client/family, physician, hospital staff and home care agency staff.
    6. f. Work closely with Weekend Manager in maintaining referral expectations within the agency’s daily and future capacity projections.
  3. Additional Responsibilities as requested.
Performance Responsibilities and Standards Field RN:
  1. Ensure quality and safe delivery of home health services.
    1. a. Registered Nurse is able to provide care to a diverse set of clients.
    2. b. Follow plan of care based on client assessment and OASIS evaluation.
    3. c. Ensure that all documentation meets quality standards:

    Completes all OASIS evaluations.
    Documentation reflects nursing care and interactions.
    Documentation is clear and concise.
    Documentation is completed in timely manner.

    1. d. Understand and follow nurse code of ethics and standards of practice.
    2. e. Coordinate with internal and external resources to meet patient needs.
    3. g. Communicate client care issues to Weekend Manager/Clinical Supervisor.
Requirements:
  1. Current license to practice as a Registered Professional Nurse in New York State or degree in a health-related field.
  2. Minimum one (1) year home health care experience preferred.
  3. Current New York State driver's license and automobile insurance required.
Physical Requirements:
  1. Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.
  2. Ability to perform tasks involving physical activity including:
    1. Lifting/ carrying - Typical weight 25 pounds.
    2. Patient transfers, extensive bending, standing, kneeling, walking in various weather conditions e.g. snow, rain, sleet, and going up and down stairs.
  3. Ability to drive in all weather conditions.
  4. Visual/hearing ability sufficient to comprehend written/verbal communication.
  5. Requires standing or walking approximately 75% of day.
  6. Hearing/speaking ability sufficient to communicate on the telephone and with families, physicians and hospital staff.
  7. Ability and willingness to drive in all kinds of weather conditions to make facility visits.
  8. Bloodborne Pathogens Exposure Category I – Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.

Competitive Salary: This position is an S03 exempt position with a min-max rate of: $70,131 -$81,275 /annually with:
