Position Summary

We are hiring Remote Nurse Practitioners to perform telehealth-based clinical assessments and follow-ups for geriatric patients. This non-prescribing role focuses on patient evaluation and ongoing care coordination.

New graduates are welcome to apply, and while no formal training is provided, clinical supervision will be available to support professional growth and ensure high-quality patient care.

Key Responsibilities

Conduct remote evaluations and follow-up assessments via secure telehealth platforms

Perform triage and assess symptoms in non-emergent cases

Follow up within 24–72 hours of discharge or provider visits

Document all patient interactions in a HIPAA-compliant EHR

Collaborate with supervising clinicians and escalate care when appropriate

Requirements

  • Comfortable working remotely and independently within a supervised structure
  • Excellent verbal and written communication skills
  • Strong organizational and time management abilities
  • Experience or interest in senior care, primary care, or chronic care management preferred
  • W-2 status with flexible scheduling (Full-Time or Part-Time)
  • Clinical supervision provided (non-prescribing role)
