Qualifications

Responsibilities

• Handle and coordinate incoming calls related to participants and services.
• Communicate effectively with nursing and therapy staff.
• Track and monitor home care scheduling.
• Assist with participant enrollment in the program.
• Participate in end-of-life care coordination.
