AltaMed Health Services in Lynwood, CA seeks a Home Care Coordinator - CNA to support and implement home care services for participants, evaluate home environments, coordinate with home caregiver vendors and family caregivers, and manage durable medical equipment.

The role requires a California CNA certification, at least 1 year of experience with frail or elderly populations, a valid driver's license, and bilingual abilities (English/Spanish/Mandarin/Cantonese preferred).

Qualifications

• Graduation from an accredited certified nursing assistant program with a current CNA certification issued by the State of California.
• Minimum of 1 year of experience practicing as a CNA or healthcare worker, and experience working with a frail or elderly population preferred.
• A driver's License is required.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

$25.00 - $25.00 hourly
