Job summary

BoldAge PACE is seeking a Homecare Coordinator to join our team in Fresno. You will coordinate, oversee, and monitor homecare services delivered by internal staff and contracted vendors to ensure alignment with care plans and regulatory expectations.

As part of the IDT, you will participate in care planning, assessments, and reassessments, monitor service delivery, and facilitate timely plan changes. A strong focus on collaboration and quality improvement is essential.

Qualifications

  • Bilingual with fluency in Hmong.
  • Experience in mentorship, lead roles, or team coordination.
  • CNA, HHA, or equivalent experience required; LPN supervision preferred.
  • 1–3 years homecare or PACE experience preferred.
  • Experience providing personal care to frail elderly adults preferred.

Responsibilities

  • Participates as an active member of the PACE Interdisciplinary Team and contributes to care planning.
  • Coordinates scheduling and delivery of authorized homecare services with staff and vendors.
  • Monitors timeliness, quality, and adherence to care plans and documentation.
  • Provides onboarding and coaching for aides; serves as liaison to contracted agencies.
  • Engages in quality, incident, and grievance management and follow-up.

JOB SUMMARY

The Homecare Coordinator is a core member of the PACE Interdisciplinary Team responsible for coordinating, overseeing, and monitoring homecare services delivered by internal staff and contracted vendor agencies. This role ensures that authorized homecare services are deliveredin accordance withparticipant care plans, regulatory expectations, and organizational policies.

The Homecare Coordinator activelyparticipatesin IDT care planning, assessment, and reassessment processes; monitors service delivery and aide performance;identifiescare gaps or changes in participant status; andfacilitatestimelycare plan modifications.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Essential Responsibilities – IDT Participation & Care Planning
  • Serves as an active and consistent member of the PACE Interdisciplinary Team.
  • Participates in initial, annual, semi‑annual, unscheduled, and Significant Change in Condition (SDR) assessments.
  • Collaborates with nursing, therapy, social work, and providers to develop and update care plans.
  • Identifies changes in participant status and initiates IDT reassessment discussions.
  • Coordinates authorized homecare services in alignment with approved care plans.
  • Oversees internal and vendor‑provided aide services.
  • Monitors timeliness, consistency, and adherence to care plant asks.
  • Serves as the primary point of contact for scheduling and coverage issues.
  • Provides direct care in home to support participant needs when necessary.
  • Serves as assist for in‑center activities or participant care if delegated by Center Leadership.
Documentation, Authorization & Compliance
  • Ensures accurate and timely completion of aide documentation.
  • Reviews records for consistency with authorized services.
  • Communicates missed care or refusals to the IDT.
  • Creates and updates homecare authorizations as needed.
Aide Onboarding, Education & Support
  • Coordinates onboarding and orientation for internal aides and resource for vendor.
  • Provides ongoing coaching and support for complex cases.
  • Identifies education gaps and facilitates training.
  • Serves as liaison between the PACE program and contracted agencies.
  • Communicates care plans, schedules, and expectations.
  • Monitors vendor performance and escalates concerns.
Quality, Incident & Grievance Management
  • Monitors and reports incidents, grievances, and aide change requests.
  • Participates in audits and quality improvement activities.
  • Supports participant satisfaction follow‑up.
EXPERIENCE AND EDUCATION:
  • Bilingual w/ fluency in Hmong
  • Experience in mentorship, lead roles, or team coordination (preferred)
  • CNA, HHA, or equivalent experience required, LPN or supervision experience strongly preferred or other related experience working within a care team model.
  • 1–3 years homecare or PACE experience preferred.
  • Experience providing personal care to frail elderly adults (strongly preferred)
PRE-EMPLOYMENT REQUIREMENTS:
  • Must have reliable transportation, a valid driver's license, and the minimum state required liabilityauto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
