AltaMed Health Services is seeking a Home Care Coordinator in El Monte, CA to support and implement home care services for participants.

The role involves evaluating safe home environments, coordinating with home caregiver vendors, and leading IDT coordination meetings. Strong leadership and communication skills are essential to identify medical and social needs and arrange appropriate home care.

Qualifications

• Graduation from an accredited certified nursing assistant program with a current CNA certification issued by the State of California.
• Minimum of 1 year of experience practicing as a CNA or healthcare worker, with frail/elderly population preferred.
• A driver's license is required.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• A minimum requirement of a valid BLS certification or higher, following AHA or Red Cross guidelines.

Responsibilities

• Coordinate home care services and participate in home visit assessments.
• Act as liaison between the IDT and home care providers and family caregivers.
• Coordinate durable medical equipment and nursing home care as needed.

Job Overview

The Home Care Coordinator is responsible for the support and implementation of home care services for participants. Provide support with the evaluation of a safe home environment through home visit assessments and coordination with home caregiver vendors and family caregivers, coordination of durable medical equipment, and nursing home care. Attends scheduled IDT and Coordination meetings. Responsible for acting as the liaison between the IDT and the home care providers, she or he should possess good leadership and communication skills. In addition, the Home Care Coordinator should be able to identify and understand participants' medical and social needs to evaluate the home care needs of participants.

Minimum Requirements

Compensation

$25.00 - $25.00 hourly
