St. Paul’s Senior Services in San Diego is seeking a part-time Healthcare Companion and Delivery Assistant to provide 1-1 daily living support, assist with transfers, ambulation, and accompany participants to medical appointments.

Under supervision of the In-Home Services CNA Supervisor, you will follow the visit schedule, document care in the EMR, help with ADLs, and ensure safety throughout appointments.

Qualifications

• Education: High School diploma or equivalent; CNA training preferred.
• First Aid and CPR training completed in person with recognized provider.
• California CNA certification when applicable; valid driver’s license required.

Responsabilités

• Consult daily with the In-Home Services CNA Supervisor to update on participant status and changes in care plan.
• Follow the assigned schedule and ensure timely arrival at visits.
• Report cancellations, incidents, and pertinent information in real time.
• Document services provided in the participant’s electronic medical record.
• Coordinate and schedule medical companion services as needed.
• Assist with ADLs (bathing, grooming, meal prep) in the home.
• Assist with transfers and ambulation.
• Provide supervision during medical appointments to ensure safety.
• Perform other duties as assigned.

This is a part-time position in our care coordination department. Typical schedule is within the hours of Monday - Friday, 8:00AM - 4:30PM.

Job Summary

Under the direction and supervision of the In-Home Services CNA Supervisor, the Healthcare Companion and Delivery Assistants responsible for providing participants with 1-1 assistance with activities of daily living, transfers, ambulation, and ensuring safety during external medical appointments.

Duties and Responsibilities

• Consults each day with the In-Home Services CNA Supervisor to be updated on St. Paul’s PACE participant’s status, changes in care plan and to confirm scheduled visits.


• Independently follow the assigned schedule, ensuring timely arrival at all scheduled visits.


• Report cancellations, unusual incidents, and any other pertinent information to the appropriate staff members in real time.


• Document services provided in participant’s electronic medical record.


• Assist with coordination and scheduling of medical companion services as needed.


• Provide any needed assistance in the home to prepare for medical appointments i.e. bathing, grooming, meal prep, etc.


• Personal ADL assistance (toileting, hygiene, dressing) as needed.


• Assists participants with transfers and ambulation


• Provide supervision throughout medical appointments as required to ensure participant safety


• Other duties as assigned

Qualifications and Requirements

• Education: High School diploma or equivalent required. If CNA, completion of formal CNA training program. Successful completion of First Aid and CPR training in person with the American Heart Association. For CNAs, current California CNA certification. Valid driver’s credentials and reliable transportation is required


• Experience: Minimum of one year’s experience working with the frail or elderly
