Qualifications

• Experience with older adults or at-home assistance preferred.
• Knowledge of caregiving practices and home care standards.

• Ability to maintain confidentiality and handle sensitive client information.
• Proficiency with email, Microsoft Office/Google Workspace, scheduling software, and electronic documentation systems.

Responsibilities

• Build trusting relationships with clients and families.
• Complete client intakes, assessments, and ongoing care evaluations.
• Develop and update individualized care plans based on client needs.
• Communicate regularly with clients, families, caregivers, healthcare professionals, and other involved parties.
• Conduct in-home visits to check on clients and ensure quality of care.
• Monitor client needs and identify changes that may require adjustments to services.
• Coordinate with the scheduling team to ensure appropriate caregiver coverage.
• Support, train, and communicate with caregivers regarding client needs and expectations.
• Address client and family concerns promptly and professionally.
• Maintain accurate client records, care documentation, and follow-up notes.
• Identify gaps in care and work with the team to find practical solutions.
• Help ensure services are provided according to company policies and applicable regulations.
• Participate in quality improvement and uphold high standards of care.
