Senior Community Care of North Carolina - PACE is seeking a Home Care Certified Nursing Assistant to support daily living activities for participants in the PACE program. Join a team dedicated to safe home environments and compassionate care, with shifts at the SCCNC Day Center and in participant homes.

The role requires NC CNA certification, a high school diploma, a valid driver's license, immunizations, and at least one year of experience with frail or elderly individuals.

Qualifications

• Unencumbered CNA certification in NC.
• High School diploma or equivalent.
• Valid driver's license and transportation.
• Medical clearance for communicable diseases and up-to-date immunizations.
• Minimum one year experience with frail/elderly.
• Ability to consult with PCS Coordinator regarding status and care plan.

Responsibilities

• Assist participants with activities of daily living and restorative care.
• Monitor health status and report changes to the PCS Coordinator.
• Assist with meals, ambulation, and daily safety in home and day-center settings.
• Support participants during day program activities and provide a safe environment.

Pay: $17.80 - $21.50

Shift: Monday-Friday (7a-9p only 8 hours required) 1 weekend a month

About the job:

The Certified Nursing Assistant is responsible for providing and assisting participants with activities of daily living and restorative/supportive care necessary to ensure the physical and health safety of the home environment. Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior Community Care of North Carolina. As assigned, works shifts in the SCCNC Day Center and provides care and services to participants and assists with the provision of a safe and effective day program

Required Qualifications:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or ability required. • Unencumbered Certification as a Nursing Assistant (CNA) in the State of NC
• High School diploma or equivalent required
• Must have a valid driver's license, proof of insurance and have means of transportation.
• Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
• A minimum of one year's experience in working with the frail or elderly population required.
• Consults each day with the PCS Coordinator (or designee) to be updated on changes in participant status, changes in plan of care, and to confirm scheduled visits.
• Personal hygiene such as skin, mouth, hair care and bathing.
• Assists participants in and out of bed assisting with ambulation.
• Assisting with prescribed exercise which participants and aide have been taught by appropriate personnel.
• Prepares meals, including therapeutic diets, assists participants with meals.
• Assist participants to the bathroom or in using commodes, bedpans, or urinals.
