Trinity Health is seeking a part-time Certified Nursing Assistant to provide comprehensive personal care at LIFE St Mary in Feasterville-Trevose, PA. The role supports elderly participants at the LIFE program with daily living activities, mobility, and companionship.

The position primarily serves at LIFE St Mary, with occasional home care. Per Diem status and a 4p-8p shift are included, and the organization emphasizes patient-centered care and team collaboration.

Qualifications

• Must be an active Certified Nursing Assistant in the state where PACE Organization is located.
• Minimum of 1 year of documented experience with frail or elderly population preferred.
• Valid driver’s license required.
• Excellent written and verbal communication skills.
• Customer-focused with ability to exercise independent judgment and adapt to change.
• Proficiency with MS Office and familiarity with email/calendar systems.

Responsibilities

• Provide personal care assistance to help participants with activities of daily living (toileting, showers, eating, grooming).
• Assist participants with physical activity, exercise, and walking.
• Provide behavioral supervision or support (orientation, reminders, diet adherence, emotional reassurance).
• Assist with general activities such as conversation, recreation, meals, and housekeeping.

Location: LIFE St Mary, 2500 Northgate Dr, Feasterville-Trevose, PA

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

This position will be housed primarily at the LIFE St Mary facility. However, occasional home care work may be required.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred
• Valid driver's license
• Superior written/verbal communication skills
• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems
• Position may require 10% local travel and use of personal vehicle.
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
