Job summary

Mercy LIFE Norristown, part of Trinity Health PACE, seeks a Personal Care Assistant to support participants at home and in our facility. You will perform essential personal care tasks, monitor health, and report changes to staff.

Requirements include a High School Diploma or equivalent, at least 1 year working with frail or elderly populations, a valid driver's license, and strong communication and organizational skills. Sign-on bonus of $2,500 with a 2-year commitment.

Qualifications

• High School Diploma or equivalent is required.
• Minimum 1 year of experience with a frail or elderly population or training through PACE upon hire if less than 1 year.
• Preferred experience in long-term care or geriatric programs.

• Conduct ongoing participant assessments and monitoring.
• Provide assistance with daily activities including toileting, grooming, and meals.
• Aid in maintaining physical activity and exercise routines.
• Identify safety hazards and respond to emergencies.

PERSONAL CARE ASSISTANT

Position Purpose:

This role impacts participants lives by providing essential personal care services and health support.

Position Details:

$2500 sign-on bonus with a 2-year commitment paid in 3 installments. Role includes both home care and facility work.

What you will do:

• Conduct ongoing participant assessments and monitoring.
• Provide assistance with daily activities, including toileting, grooming, and meals.
• Aid in maintaining physical activity and exercise routines.
• Identify safety hazards and respond to emergencies.
• Assist with recreational activities, meal service, and housekeeping.
• Report any changes in participant's condition to center staff.
• Escort participants to appointments and assist with transportation.

Minimum Qualifications:

• High School Diploma or equivalent.
• Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization upon hiring.
• Preferred experience in long-term care or geriatric programs.
• Strong communication and organizational skills.
• Valid driver's license, insurance and use of personal vehicle.
• Ability to work independently and adapt to change.
• Some travel within the region may be required.
