Job summary

Old Mission Advanced Care seeks a wound care clinician to join a defined local pod, delivering in-home wound care and advanced therapies to homebound patients. You will work on scheduled days, with a nurse navigator coordinating care, and dispatch routing supporting your daily visits.

Credentialing completes before first visit, with a local license in Texas or Florida. This field-based role offers PRN hours, with potential conversion to full-time W-2 as pods mature and census grows.

Qualifications

• Active, unencumbered NP or PA license in TX/FL, with CAQH profile (credentialing must complete before first visit)
• Local pod territory residency (home-based route, not travel)
• Bilingual Spanish/English required for patient communication
• Procedural confidence in sharp debridement and multi-layer compression per protocol

Responsibilities

• Provide in-home wound care within assigned pod, including debridement and advanced therapies
• Manage a standing panel on a set weekly rhythm with continuity of care
• Document visits the same day with wound measurements, treatment and products used
• Escalate red-flag findings to National Clinical Director and coordinate with Nurse Navigator
• Pod growth: full-time conversion when census reaches ~28–30 patients
