Job summary

Homeside Rehab in Sundown, NY provides one-on-one home-based Physical Therapy services. We seek a licensed Physical Therapist or Physical Therapy Assistant to treat patients in their homes, evaluate needs, and implement personalized plans of care.

The role emphasizes compassionate, independent work with flexible scheduling and a supportive clinical team. Prior home health or outpatient experience is preferred, with competitive hourly compensation.

Qualifications

• Active New York State Physical Therapist (PT) or Physical Therapy Assistant (PTA) license/certification is required.
• Basic computer proficiency for electronic documentation.
• Strong interpersonal and communication skills are essential.
• Ability to work independently in a home-based setting.

Responsibilities

• Provide skilled Physical Therapy services in the patient's home.
• Evaluate patients and develop individualized plans of care.
• Carry out established treatment plans and interventions.
• Provide functional activities, therapeutic exercises, ADL training, strengthening and coordination activities.
• Monitor patient progress and communicate changes to supervising therapist and clinical team.
• Maintain accurate and timely clinical documentation per regulatory requirements.
• Communicate professionally with patients, caregivers, therapists, and office staff.
• Provide one-on-one therapy services in the patient's home.
