Qualifications

• Appropriate licensure in Missouri required.
• Bilingual preferred but not required.

Responsibilities

• Deliver comprehensive primary care at home visits to patients.
• Represent Bloom Healthcare in the St Louis market to support market growth.
• Plan to grow patient panel with ~10 visits per day.
• Coordinate with care teams and facilities to ensure optimal outcomes.
• Engage with population health data to drive improvements.
