Job summary

The U.S. Department of Veterans Affairs seeks a Home Based Primary Care Registered Nurse to deliver comprehensive, patient-centered home care for Veterans across the VA St. Louis Health Care System, including multiple Missouri and Illinois sites and rural areas.

You will coordinate with an interdisciplinary team, focus on education and self-management, and participate in quality improvement to achieve optimal health outcomes for Veterans.

Qualifications

• English language proficiency is required for direct patient care.
• Graduate of an approved nursing program and properly licensed or eligible for licensure.
• Current, full, active, and unrestricted RN registration in the U.S. or District of Columbia.

Responsibilities

• Deliver and coordinate home-based, patient-centered care to Veterans.
• Coordinate with interdisciplinary team to support education and self-management.
• Perform case management, assessment, triage, and develop individualized care plans.
• Provide guidance to licensed practical nurses and nursing assistants as appropriate.
• Contribute to quality improvement initiatives and ongoing evidence-based practice.
