Job summary

Neighborhood Health Plan of Rhode Island and Health@Home collaborate to deliver home-based primary care to members with complex medical and social needs in Rhode Island.

The Nurse Practitioner will conduct comprehensive home visits, develop and update individualized care plans, coordinate with PCPs and the care team, and educate members and families to promote safety and independence, aiming to reduce hospitalizations.

Qualifications

• APRN license active in Rhode Island.
• Graduate of an accredited NP program.
• Experience delivering primary care to adults in underserved populations.
• Comfortable with home-based care delivery.
• Strong documentation and organization skills.
• Excellent oral and written communication.
• Self-motivated and dependable with transportation.

Responsibilities

• Perform 4–6 home visits per day with an individualized care plan.
• Develop and update comprehensive care plans with the care team.
• Monitor chronic conditions and assess home environment and social determinants.
• Educate members and caregivers on safe care practices.
• Collaborate with PCPs, nurse practitioners, case management and behavioral health.
• Prescribe and adjust medications as appropriate.
• Document encounters and update EMR within required timeframes.
• Participate in on-call rotation and ensure timely responses.
