Job summary

The U.S. Department of Veterans Affairs is seeking a Nurse Practitioner to join Home Based Primary Care.

The NP delivers advanced clinical practice, consultation, and care management for Veterans in a home-based program, with emphasis on quality, timeliness, and the VA mission. As an independent practitioner, you will manage a panel of patients with acute and chronic conditions, order labs, prescribe therapies, and develop individualized plans to improve health outcomes.

Qualifications

• Licensed independent practitioner with advanced education and national certification.
• Evidence-based care for a patient population.
• Strong ability to develop and implement care plans and to lead interdisciplinary collaboration.

Responsibilities

• Independently manages a panel of patients with acute and chronic conditions.
• Orders and interprets labs, tests, and diagnostic studies.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals to specialty services and provides patient-family education.
• Demonstrates ICARE values in daily practice.
