Job summary

The Minneapolis VA Health Care System is seeking a Home Based Primary Care APRN who will perform physical assessments, engage in therapeutic problem solving, counseling, consultation, and provide collaborative case management with an interdisciplinary team.

The APRN will prescribe medications and therapies within the guidelines of the practice agreement, teach patients and families, triage, and coordinate care with other disciplines.

Qualifications

• BLS certification required.

Responsibilities

• Performs physical assessments and engages in therapeutic problem solving, counseling, and consultation.
• Provides independent case management and collaborates with physicians and health care team.
