Responsibilities

• Provide home-based intake and assessment to tailor services and treatment plans.
• Develop a service plan from DePelchin and community resources addressing assessed needs.
• Assist with referrals and advocate for client entitlements or needed services.
• Maintain current records and meet agency reporting requirements.
• Track monthly statistics for program accountability and billing.
• Collaborate with staff to provide community education and presentations.
