Job summary

Virtua Health's Virtua Medical Group is seeking an experienced Nurse Practitioner to join our Home-Based Palliative Care team. You will perform home visits across the community, delivering high‑quality, patient‑centered care to individuals with serious, chronic, or life‑limiting illnesses.

You will work with physicians, nurses, social workers, hospice partners, and other team members to develop individualized care plans.

Qualifications

• Experience in palliative and hospice care required.
• Experience delivering home-based primary care or house calls.
• Ability to manage advanced chronic illnesses and complex symptoms.

Responsibilities

• Provide home-based palliative care visits coordinating with physicians, nurses, social workers and hospice partners.
• Develop individualized care plans aligned with patients' goals.
• Support patients and families through complex medical decisions and transitions of care.
