Sobe Innovative Rehab in Polk City, FL is seeking a licensed Occupational Therapist to provide outpatient therapy in a home setting on a PRN basis. Qualifications

• Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program
• Active Florida Occupational Therapist (OT) license

• Develop and implement individualized treatment plans
• Collaborate with physicians, families, and the interdisciplinary care team
• Maintain accurate, timely documentation in the EMR
