Job summary

Homeside Rehab provides one-on-one home-based Occupational Therapy services in Carle Place, NY 11514. Our team delivers skilled OT/OTA care directly at patients’ homes, supporting independence and functional goals.

Qualifications

• Active NY OT or OTA license/certification
• Basic computer proficiency for electronic documentation
• Strong interpersonal and communication skills
• Ability to work independently in a home-based setting
• Valid driver's license and reliable transportation for home visits
• Home health, outpatient, rehabilitation, or skilled nursing experience preferred
• Minimum of 1 year of clinical experience preferred

Responsibilities

• Provide skilled Occupational Therapy services in the patient's home
• Evaluate patients and develop individualized plans of care
• Carry out established treatment plans and therapeutic interventions
• Provide functional activities, therapeutic exercises, ADL training, strengthening, coordination activities
• Monitor patient progress and communicate changes or concerns to the supervising therapist
• Maintain accurate and timely clinical documentation
• Communicate professionally with patients, caregivers, therapists, and office staff
