Job summary

You will assess, treat, and educate patients, supervise OT assistants when needed, and coordinate with the rehabilitation team to customize plans of care. A valid OT license and reliable transportation are required.

Qualifications

• Must have a valid Occupational Therapy license in the state of practice.
• Must have a valid driver's license or reliable transportation to patients' residences.

Responsibilities

• Provide outpatient rehabilitation services in-home for older adults.
• Supervise OT Assistants, temporary licensees, students and support personnel as allowed.
• Adhere to treatment schedules and complete clinical and billing documentation.
• Attend care conferences and coordinate with other rehab team members.
• Adapt plan of care to individual patient needs and maintain a safe environment.
• Assess, evaluate, and document patient progress and outcomes.
