Job summary

Health Care Administrators Association seeks a Nurse Practitioner to provide home-based primary care for medically complex, homebound adults. You will conduct in-home and telehealth visits, manage chronic and acute conditions, and coordinate with an interdisciplinary team to optimize outcomes.

You will order tests, prescribe medications as appropriate, and educate patients and caregivers to prevent hospitalizations.

Qualifications

• Current Nurse Practitioner licensure and clinical experience.
• Ability to manage multiple conditions in a home-based setting.
• Proficient with telehealth and electronic health records.

Responsibilities

• Perform in-home and telehealth visits to assess patient needs.
• Manage chronic and acute conditions through comprehensive evaluations.
